Pathfinder 2e

Common Conditions

Pin status, circumstance, movement, and action reminders.

Same-type penalties do not add: apply the highest relevant value. For example, frightened 2 and sickened 1 impose a −2 status penalty, not −3.

These cards cover frequent conditions; the source links provide the complete list.

Clumsy

Status penalty equal to the value on Dexterity-based checks and DCs, including AC, Reflex saves, and relevant attacks and skills.

Concealed / Hidden

Concealed targets require a DC 5 flat check; hidden targets a DC 11 flat check. These are separate from the attack roll. Hidden also makes you off-guard to that creature.

Drained

Status penalty equal to the value on Constitution-based checks. Lose HP equal to level × value and reduce maximum HP by that amount; see recovery rules.

Source ↗

Enfeebled

Status penalty equal to the value on Strength-based checks and DCs, including Strength-based damage rolls.

Source ↗

Frightened

Status penalty equal to the value on all checks and DCs. Decreases by 1 at the end of each turn unless otherwise specified.

Source ↗

Grabbed

Off-guard and immobilized. Manipulate actions require a DC 5 flat check or the action is lost.

Source ↗

Off-guard

−2 circumstance penalty to AC. Sometimes applies only against specific creatures or attacks.

Source ↗

Persistent damage

Take the damage at the end of each turn, then attempt the recovery flat check, normally DC 15. Assistance can improve the check or end the condition; use the effect’s rules.

Source ↗

Prone

Off-guard; −2 circumstance penalty on attacks. Crawl or Stand for movement. Taking Cover while prone can give greater cover against ranged attacks.

Source ↗

Quickened

Gain one additional action when your turn starts, restricted to the actions the effect permits. Multiple sources do not grant additional extra actions.

Source ↗

Restrained

Off-guard and immobilized. Only Escape or Force Open actions can have attack or manipulate traits while restrained.

Source ↗

Sickened

Status penalty equal to the value on all checks and DCs; cannot willingly ingest things. Spend an action retching to attempt a Fortitude save against the original DC to reduce it.

Source ↗

Slowed

When regaining actions at the start of a turn, reduce that number by the slowed value. It does not retroactively remove actions already gained.

Source ↗

Stunned

A value counts actions lost across turns. Stunned overrides slowed; account for actions lost to stunned when also slowed. Some effects specify a duration instead of a value.

Source ↗

Stupefied

Status penalty equal to the value on Intelligence-, Wisdom-, and Charisma-based checks and DCs. Casting a spell requires a flat check of DC 5 + value or it is disrupted.
